Practice Plus Group are Health in Justice Market leader, providing healthcare services to offenders across the country including HMP Ford, a category D prison, also known as a resettlement or open prison for adult men near Arundel in West Sussex.
Our model of care is our Wellbeing Approach – an integrated, comprehensive approach in recognition of the fact prisoners are a socially-excluded group with healthcare needs which are complex and often associated with poor outcomes. It adopts the premise no single health/wellbeing factor can be taken in isolation of other motivational issues which might impact a person’s care.
What do we need from you so you can do the job?
As a Primary Care Team Leader you will be a registered general nurse, ready to provide leadership to a team of nurses and other health professionals. You will have knowledge and experience of managing long term conditions and health promotion and be able to share that knowledge with the wider team to provide a high class healthcare service to the resident population.
You will be providing appointment based face to face, safe and effective health assessments and treatments as well as responding to emergencies, minor injuries and leading on the reception and discharge of patients.
You'll be responsible for the rota management of the primary care team, their clinical supervision and line management. You will work closely with other team leaders, healthcare managers and prison colleagues to deliver health services in an innovative and proactive way to meet the requirements of the patients, HMPPS, NHS England, Public Health and Practice Plus Group.

This is a full-time role working 37.5 hours per week. The services are provided seven days a week. Please note that offers of employment are subject to Practice Plus Group Terms & Conditions and receipt of satisfactory references, an enhanced DBS check and prison vetting check.
